DNN Module unavailable after form submittal

Hello there,

I have been using this module for the past 2 weeks at my job and it has been working just fine. For some reason this weekend, we received the attached error after hitting "Send Feedback" We were receiving an initial error that was server related when we first added the module to our site, but we have since fixed this issue. I have hidden it from view on our site for the time being in order to continue troubleshooting.

Has anyone encountered this random error, and if so, do you have a solution? Or know why it is happening?

You should see an error in the event log as well, can you provide that?

I'm seeing this error repeated almost 200 times in this log:

This looks to me like you have some other issue on your site. I don't believe that error is generated by the module. It looks like it is some sort permissions error...

Thanks. I did a little more digging around and I think I'm coming to that conclusion too. If I figure it out, I'll post my solution in the event someone else comes up with this crazy error.
